- SMS Text Messaging Terms & Conditions Program description. California Vascular & Interventional PC (“we,” “us,” “CVI”) operates an SMS text messaging program for patients and prospective patients who have opted in. Depending on the consent you provide, you may receive: (a) marketing and promotional messages, including special offers, new service and treatment announcements, screening event invitations, and general health education; and/or (b) appointment-related messages, including confirmations, reminders, scheduling changes, and follow-up instructions. Marketing consent and appointment consent are collected separately, and neither is required to receive the other. Marketing messages do not contain protected health information or details about your individual care. Eligibility. You must be 18 years of age or older and the subscriber or authorized user of the mobile number you provide to enroll in the SMS program. How to opt in. You enroll by submitting your mobile number through a form on cavascular.com and affirmatively checking the applicable consent checkbox. The checkbox is not pre-selected, and consent is not a condition of purchase, of submitting the form, or of receiving treatment. How to opt out. You may cancel the SMS service at any time by replying STOP to any message you receive from us. After you reply STOP, we will send you one confirmation message and will not send further text messages unless you opt in again. You may also opt out by calling (310) 409-4114. How to get help. For help or more information about the program, reply HELP to any message, call (310) 409-4114, or contact us at https://cavascular.com/contact-us/. Message frequency. Message frequency varies. You can expect approximately 2-4 messages per month, depending on the programs you have opted in to. Message and data rates. Message and data rates may apply. These charges are billed by and payable to your mobile service provider. Please consult your mobile service provider’s pricing plan to determine the charges for sending and receiving text messages. We are not responsible for any charges imposed by your carrier. Carrier liability. Carriers are not liable for delayed or undelivered messages. Delivery of messages is subject to effective transmission by your mobile service provider and is outside our control. We do not guarantee that messages will be delivered, and we are not liable for any delay, failure to deliver, or misdirected delivery of any message, or for any errors in the content of any message. Supported carriers and changes. The SMS program may not be available on all mobile carriers, and supported carriers may change without notice. We may modify or discontinue the SMS program, or any of these terms, at any time without prior notice. Privacy. Your mobile number and SMS consent are handled as described in our Privacy Policy at https://cavascular.com/privacy-policy/. No mobile information will be shared with third parties or affiliates for marketing or promotional purposes, and text messaging originator opt-in data and consent are never shared with any third party.
